A Development of Dental Service System in Type-2 Diabetic Patients at Kongra Hospital, Phatthalung Province

บทคัดย่อ
การวิจัยเชิงปฏิบัติการนี้มีวัตถุประสงค์เพื่อพัฒนาระบบบริการทันตกรรมในผู้ป่วยเบาหวานชนิดที่ 2 โรงพยาบาลกงหรา จังหวัดพัทลุง ดำเนินการวิจัยระหว่าง 1 ตุลาคม พ.ศ. 2560 ถึง 30 เมษายน พ.ศ. 2561 การวิจัยแบ่งเป็น 4 ระยะ คือ 1) ระยะสำรวจปัญหา 2) ระยะออกแบบระบบบริการทันตกรรม 3) ระยะทดลองใช้ระบบบริการทันตกรรม และ 4) ระยะประเมินผล คัดเลือกกลุ่มตัวอย่างตามเกณฑ์การคัดเลือก ประกอบด้วย ผู้ป่วยโรคเบาหวานชนิดที่ 2 จำนวน 15 คน ทีมสหวิชาชีพผู้ให้บริการ จำนวน 11 คน และผู้มีส่วนได้ส่วนเสียในการนำข้อมูลไปใช้ จำนวน 2 คน เก็บข้อมูลเชิงคุณภาพโดยใช้เทคนิคการสัมภาษณ์แบบเจาะลึกและนำบทสัมภาษณ์ไปถอดข้อความจากเทปแบบคำต่อคำและตรวจสอบความถูกต้องของข้อมูลด้วยเทคนิคสามเส้า วิเคราะห์ข้อมูลโดยใช้ การวิเคราะห์เนื้อหา เก็บข้อมูลเชิงปริมาณโดยใช้แบบตรวจสภาวะปริทันต์ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ผลการวิจัยในมุมมองของตัวอย่างพบว่า สถานการณ์โรคเบาหวานชนิดที่ 2 ในพื้นที่อำเภอกงหรา จังหวัดพัทลุง มีแนวโน้มผู้ป่วยและความรุนแรงของโรคเพิ่มขึ้น ระบบบริการทันตกรรมเดิมของโรงพยาบาลกงหราทำให้ผู้ป่วยเบาหวานชนิดที่ 2 เข้าถึงยากและให้บริการเชิงตั้งรับ จึงพัฒนาระบบบริการทันตกรรมใหม่โดยอาศัยความร่วมมือกันระหว่างผู้ป่วยเบาหวานที่มารับบริการและผู้ให้บริการ โดยกำหนดกิจกรรมการดำเนินงานอย่างต่อเนื่อง และสอดคล้องกับบริบทและความต้องการของผู้ป่วยเบาหวานที่มารับบริการทันตกรรมภายใต้ศักยภาพของหน่วยบริการ ซึ่งระบบบริการทันตกรรมที่พัฒนาขึ้น ประกอบด้วย เปิดช่องทางพิเศษเพื่อนัดหมายผู้ป่วยเบาหวานชนิดที่ 2 เข้ารับบริการทันตกรรมและปรับปรุงสมุดประจำตัวผู้ป่วยเบาหวาน รวมทั้งโทรศัพท์เตือนนัดหมาย ส่งผลให้ผู้ป่วยกลุ่มนี้เข้ารับบริการทันตกรรมเพิ่มมากขึ้น และสภาวะปริทันต์ของผู้ป่วยเบาหวานชนิดที่ 2 ดีขึ้น
