บทคัดย่อ
งานวิจัยนี้มีวัตถุประสงค์เพื่อการพัฒนาระบบบริการไร้รอยต่อ โรงพยาบาลพาน อำเภอพาน จังหวัดเชียงราย ระยะเวลาดำเนินการตั้งแต่เดือนตุลาคม พ.ศ. 2557 ถึง เดือนกันยายน พ.ศ. 2562 ผู้ให้ข้อมูลมี 2 กลุ่มคือ 1) ทีมพัฒนาบริการ ได้แก่ พยาบาล นักกายภาพบำบัด นักโภชนากร รวม 22 คน และผู้บริหาร 3 คน 2) กลุ่มผู้รับบริการ ได้แก่ อสม. ผู้นำชุมชน และผู้นำท้องถิ่น รวม 26 คน และผู้ป่วย ผู้ดูแล รวม 53 คน ซึ่งเป็นผู้ใช้บริการที่โรงพยาบาลพาน โดยระบบบริการไร้รอยต่อ โรงพยาบาลพาน ประกอบด้วย 1) ระบบบริการไร้รอยต่อ 5 ขั้นตอน เป็นการทำงานร่วมกับทีมสหสาขาวิชาชีพ 2) ระบบส่งต่อข้อมูลและให้คำปรึกษาทางวิดีโอคอลและ แอปพลิเคชันไลน์ 3) ระบบการวางแผนจำหน่ายผู้ป่วย ที่ใช้งานร่วมกันระหว่างบุคลากรในโรงพยาบาล โรงพยาบาลส่งเสริมสุขภาพตำบล ศูนย์สุขภาพชุมชน และผู้ดูแล และ 4) ระบบสนับสนุนวัสดุ อุปกรณ์ที่จำเป็นในการดูแลผู้ป่วยที่บ้าน ที่เอื้อต่อการเบิกจ่ายยาและเวชภัณฑ์เพื่อการดูแลผู้ป่วย โดยมีผู้จัดการรายกรณีและคณะกรรมการทำงานการดูแลที่บ้านของโรงพยาบาล เป็นตัวเชื่อมผ่านแนวคิดการจัดการความรู้และการจัดเวทีแลกเปลี่ยนเรียนรู้ จากการดำเนินการพบว่า จำนวนผู้ป่วยหอผู้ป่วยนอกลดลง ลดค่าใช้จ่ายของผู้ป่วย ผู้ป่วยได้รับบริการดุจญาติมิตร ทำให้เกิดความพึงพอใจและผูกพันกับโรงพยาบาล และทำให้บุคลากรทางสุขภาพมีแนวทางการพัฒนางานประจำสู่งานวิจัยได้
